Payment
We participate with Medicare, Blue Cross/Blue Shield, the Empire Plan, and many other commercial insurances. Please provide us with your most current insurance card. Many insurances require the patient to pay a co-pay for office visits and x-rays. Please be prepared to pay your co-pay on the day of service. We do not bill for co-pays. If you are not covered under insurance, payment will be due at the time of service. If you have a financial situation which causes your medical expenses to be an undue burden, we will be pleased to discuss a payment plan with you. We will bill your primary and secondary insurances, and bill you for the portion of the bill that is your responsibility once the insurance has paid. Please note: Fees for X-ray and Physical Therapy will be billed separately by our in-house ancillary department, H.S.S. Ancillary Services, P.C. Fees for medical supplies such as crutches, wrist, ankle and knee splints will also be billed separately by DonJoy Office Care.
